Located in Columbia, Tennessee, Columbia CARES serves as the region’s only AIDS-Service Organization in the South-Central Region of Middle Tennessee.
Columbia CARES was established in 1992 in response to the HIV/AIDS epidemic to provide support for people living with HIV/AIDS, as well as their families and friends, and to provide prevention education to people at risk for HIV infection. Our focus is education, prevention, testing, and social services for people affected by HIV/AIDS.
We are seeking an Executive Director to continue our role by consistently supporting the established mission and overall purpose of the organization and contribute to its strategic direction to ensure a successful future.

Position Title: Case Manager
Description: Full Time/Salaried
Reports To: Executive Director
Education: Bachelor in Health/Human Service preferred or years equivalent in case management experience.
Case Management Responsibilities
*Complete Client Intake Interviews & History.
*Complete a needs assessment of client circumstances upon initial contact to identify present needs and to anticipate future needs.
*Outline a plan of care with goals, objectives and timelines which the client participates in developing.
*Assist each client in obtaining access to resources needed to meet activities of daily living including but not limited to medical care, Alcohol & Drug treatment, food, clothing, shelter, counseling therapy, education, job training & skills enhancement, financial planning, legal counsel, financial aid & entitlements and other support services.
*Conduct home visits or arrange office appointments on each client/family.
*Serve as a client advocate to meet identified needs.
*Provide referral networking and coordinate interagency services for clients and families. *Serve as a liaison and consultant in collaborating with other social service providers & agencies involved as members of the client's treatment team.
*Maintain thorough, updated client files.
*Maintain data base information including demographics and other statistical data.
*Activities which retain clients in HIV primary care and social services. *Activities to recruit previous clients out of care.

Community-based case management is a collaborative process that promotes family independence and the capacity to meet an individuals special needs. A comprehensive plan to address both individual and family needs is developed based upon identified priorities, concerns and resources. Case Management is also referred to as care coordination or service coordination. Limited travel within the 12-county service area with mileage reimbursement at the Federal Mileage Reimbursement rate. (Bedford, Coffee, Giles, Hickman, Lawrence, Lewis, Lincoln, Marshall, Maury, Moore, Perry, and Wayne)
Position Requirements
-Bachelor in Health/Human Service preferred or equivalent years of experience in similar field
-Reside within 30 miles of agency location in Columbia, TN
-Must possess excellent communication skills
-Ability to adapt in various situations by exercising judgement
-Proficiency in Microsoft Word, Excel, Internet, and Email applications

Columbia CARES PrEP Navigation Program works in accordance with standards consistent with the Centers for Disease Control & Prevention and Tennessee Department of Health HIV PrEP Guidelines.
PrEP navigation is a collaborative process that promotes HIV prevention strategies, specifically PrEP. A comprehensive plan to address HIV prevention is developed with the client based upon identified priorities, concerns and resources. Limited travel within the 12-county service area with mileage reimbursement at the Federal Mileage Reimbursement rate. (Bedford, Coffee, Giles, Hickman, Lawrence, Lewis, Lincoln, Marshall, Maury, Moore, Perry, and Wayne).

*Recruit and maintain an active caseload of PrEP clients
*Provide support and guidance to all referrals seeking PrEP services
*Perform assessment plan
*Navigate clients through the necessary steps to acquire PrEP
*Assist to eliminate barriers to access PrEP
*Educate clients and others about HIV disease process and prevention
*Acquire and complete necessary paperwork
*Maintain client charts
*Submit required reports
*Assist in the promotion of the program-social media campaigns
*Communicate with medical providers and their support staff
*Perform other duties as assigned by the Executive Director

-Reside within 30 miles of agency location in Columbia, TN
-Must possess excellent communication skills
-Social media experience
-Proficiency in Microsoft Word, Excel, Internet, and Email applications

Primary Duties Include
Fiscal Management of Agency
- Oversight of $375,000.00 annual budget
- Process monthly invoices and reconcile all agency expenses
- Maintain accurate financial records and paperwork as required by state, funders, and/or other necessary statutory regulations
- Provide monthly budget report to Board of Directors and inform Board Officers of any outstanding issues or needs that require immediate attention.
Funding Management of Agency
- Serve as grant writer, completing any necessary contract applications or reapplications
- Internal monitoring of all funding to ensure compliance with funding agency
- Identify revenue resources, including fundraising opportunities
- Audit / report preparation for funding sources as requested and/or required
Administrative and Organization Management
- Supervision of full-time and part-time staff ranging from 4-6 employees.
- Prepare, maintain, and process payroll for staff
- Responsible for day-to-day operations of office, including appropriate resource management (ensure agency bills/taxes are paid/filed in a timely manner)
- Ensure organization is appropriately staffed based on available funding and workload.
- Ensure staff are trained on job responsibilities.
- Serve as client / consumer advocate and assist with case management
- Provide testing and counseling as needed
- Serve as agency contact for consumer financial assistance
Liaison to Community
- Maintain and develop relationships with other non-profit agencies and provide representation in the community as needed.
- Provide education to community organizations as requested
- Serve as consumer advocate for rural to extreme-rural communities
Additional Responsibilities:
- Limited travel within the 12-county service area may be required with curred Federal Mileage Reimbursement provided. (Bedford, Coffee, Giles, Hickman, Lawrence, Lewis, Lincoln, Marshall, Maury, Moore, Perry, and Wayne)
- Coordination of annual fundraising events including planning, marketing, and required preparations.
- Coordination and communication of monthly Board of Directors meeting
- Manage the organization’s social media pages and keep the community informed
Position Requirements
- Must live within 30 miles of agency location in Columbia, TN
- Must possess excellent communication skills and the ability to handle stressful situations with tact
- Ability to adapt in various situations by exercising judgement and adjusting to unexpected challenges.
- Proficiency in Microsoft Word, Excel, Internet and E-mail applications, experience with publishing/graphics programs, and basic office computing troubleshooting.
- Ability to manage time and workload effectively to meet responsibilities
- Previous supervisory or project management experience required
- Previous experience with fundraising and event planning preferred
- Previous experience with writing grants, contracts, and/or legal agreements required
